To request a copy of your mental health records or to give another person access to your records, students can submit an Authorization for Release of Medical and Mental Health Information online, by fax, by mail, or in-person. However, many of your records are already available on the The Ashe Center Patient Portal and do not require prior authorization to view.

For Records Prior to July 8th, 2025

Current and previous students requesting their records for treatment records prior to July 8, 2025 can submit an Authorization for Release of Medical and Mental Health Information online, by fax, by mail, or in-person.

Once received, a member of CAPS Support Staff will confirm receipt and will ask how you'd like the records to be sent out. Please note that CAPS can only fax, mail, and have records ready for pick up in person. We are not able to email records as this is not a secure form of communication. It is also important to note that legally we have 14 business days to process your records.

For Records from Appointments that Occur On or After July 8, 2025

There are several options that you can choose to request your records:

Option 1: Request mental health records via your MyStudentChart account

If you have not signed up for MyStudentChart, go to How to Sign Up for MyStudentChart for instructions.

1. Log into MyStudent Chart portal

  • (Request for records can only be accessed via PC, mobile devices are not supported as this time).

2. Follow instructions using links below:

Option 2: Download and print the authorization for Release of Health Information form.

1. Download and print the authorization form for Release of Health Information for Patient or a third-party (i.e. Non-UCLA Provider, Insurance Company, Attorney, etc.)

2.Complete and sign the form.

3. Submit completed form via email, fax, mail, or in person.
